Participation Group and to also have
a 'virtual' Patient Participation Group
- this is an email community that we
can consult on a regular basis about
our services.We would like our 'virtual
group' to be representative of our local
community and to include patients of
all ages and with a wide variety of
different needs. If you would like to
be involved we would be very grateful
if you could provide your email address.
We can then contact you by email
every now and again to seek your opinion.
We will only ask questions about the
Practice and these questions will be
via short questionnaires.If you are
happy for us to contact you periodically
by email, please complete the 'invitation'
form which can be found on our website
under 'our documents' section, or you
can pick up a form from reception.
We will add your email address to a
contact list. Your contact details will
be kept safely and securely and will
only be used for this purpose. If you
decide at any point that you no longer
wish to be involved, please inform us
in writing and we will be happy to
remove your contact details.
Alternatively, if you would like to be part
of the Patient Participation Group which
meets from time to time in the surgery
please complete the Patient Participation
Group leaflet found on our website under
'our documents' secion, or you can pick
up a form from reception. We will be in
contact with your further once you have
expressed your interest.
We now have Miss Michealla Hunt
our fully qualified Phlebotomist and Assisstant Practitioner
She holds a blood clinic every
Thursday morning at the surgery.

AND PASSWORD CAN BE OBTAINED FROM
THE SURGERY TO ACCESS THIS SERVICE.
